MEMORANDUM
Seneca, Allegany, Livingston, Yates, Chemung, Broome, Cortland and Tioga counties .
Members of the ADHCC in the counties of Wayne, Ontario, Steuben, Cayuga, Seneca, Allegany, Livingston, Yates, Chemung, Broome, Cortland and Tioga are invited to a transportation meeting with representatives of the Department of Health (DOH) on Wednesday, April 25, at Waterloo ADHC in Waterloo, NY.
The purpose of the meeting is to finalize a regional transportation reimbursement rate effective January 1, 2008, for programs that elect to manage their own transportation.
Members should make every effort to attend this meeting. All ADHC programs are invited to attend regardless of whether the cost of transportation is currently included in your ADHC rate or your transportation is provided through the local department of social service (LDSS). Program directors as well as individuals from the sponsoring facility’s finance office and/or administrative offices are welcome.
Attached you will find directions to Waterloo ADHC, and a response form to be returned by Friday, April 20, 2007.

REVISED Directions to Waterloo ADHC
From the East:
Take Interstate 90 to Exit 41 (Waterloo exit).
Turn right onto Route 414 South. (414S will eventually pass a Kentucky Fried Chicken on the right, followed by a Holiday Inn.)
Bear right onto Routes 5 & 20 West just past a McDonald’s.
Two-tenths of a mile past a Wal-Mart on the right, turn right into the Huntington Living Center*; Waterloo ADHC will be straight ahead.
To park, drive past the senior apartments on the left and then park in the lot to your left.
Just inside the main entrance of the Taylor Brown Health Center, the conference room will be on the left.
From the West:
Take Interstate 90 to Exit 41 (Waterloo exit) and then follow the above directions.
From the South:
Take Route 414 North to Route 96 North.
Turn right onto Routes 5 & 20.
Drive 1.5 miles and turn left into the driveway of the Huntington Living Center; Waterloo ADHC will be straight ahead.
To park, drive past the senior apartments on the left and then park in the lot to your left.
Just inside the main entrance of the Taylor Brown Health Center, the conference room will be on the left.
From the North:
Take Route 414 South to Routes 5 & 20 and then follow the above directions from the east.
*Huntington Living Center is located at 369 East Main Street, Waterloo, 13165. In front of the building, there is a large green and white sign with a red hearpple. (A hearpple is a combination of a heart and an apple.)
